Apprentice Application
- Next General Monthly Meeting is March 21st Wednesday at the Union Hall start time 7:00 PM

Some members on rotation have been asking about the UI-UC348 forms they are receiving in the mail.  
Sorry I am new to this form but this is the form that replaced the UC-226 forms that the union used to sign
and is the replacement for anyone on partial or rotation. This form is to be completed by your employers.

UC-348 Partial Unemployment Information (Information for claimants and employers on the definition, assessment and determination of
partial unemployment. Also, includes the “Verification of Partial Unemployment Status” form for employers to compete and return to confirm
their employee’s partial unemployment status. This form is due within 5 working days from the date the application is filed.)

As for the UC-226 the union signs these are only turned in when you are completed released by the company and verifies the union will be
referring work to you.  

UC-226  Verification of Registration with a Referring Union.  If you report that you will obtain work through a referring union, have an
authorized union official complete and submit this form to your local claims office within 7 calendar days after filing your claim.
